| Abstract | 「はじめに」 心不全に対する薬物治療は, ここ数十年の大規模臨床試験の結果から目まぐるしく発展してきた. しかしながら, 高齢化の進行や併存疾患の多彩さによって重症心不全の数も激増している. 様々な薬物が使用できる反面, その使い方や組み合わせ方も多彩で, エビデンスに応じた治療戦略が要求されている. 本稿では特に, 重症心不全に対する治療薬の使い方に焦点を絞って概説する. 「心不全を取り巻く状況」 わが国における心不全患者の総数は高齢化の進行とともに年々増加しており, 2010年には100万人を突破し, 2030年には130万人を超えると予想されている. 生命予後を悪性腫瘍と比較してみると, 軽度〜中等度の心不全の予後は胃がんの予後と同じような経過をたどり, 中程度〜重度の心不全の予後は肺がんや結腸がんなどと同等に不良である. わが国における急性心不全による入院日数と院内死亡率は2010年代になって低下傾向にあるものの, 1年死亡率はむしろ増加傾向にある. |
