| Abstract | 「はじめに」 サクビトリルバルサルタンはアンジオテンシンII受容体拮抗薬(ARB)であるバルサルタンと, ネプリライシン阻害薬のプロドラッグであるサクビトリルの化合物である. 左室駆出率(LVEF)が低下した慢性心不全症例を対象としたPARADIGM-HF試験において, サクビトリルバルサルタンはアンジオテンシン変換酵素(ACE)阻害薬であるエナラプリルと比較して, 心血管死および心不全増悪による入院からなる複合エンドポイントを有意に減少させた. この試験結果は2014年に報告されているが, 日本人を対象とした同様の試験であるPARALLEL-HF試験を経て, ようやく本邦でも2020年8月より使用が可能となり, 徐々に処方数が増加しつつある. 海外における第III相試験であるPARADIGM-HF試験および国内第III相試験であるPARALLEL-HF試験ではサクビトリルバルサルタンの主な副作用として, 低血圧が10.4%の症例に, 高カリウム血症が4.7%, 腎機能障害が2.9%, 腎不全が0.8%, 血管浮腫が0.2%に認められた. |
