| Abstract | 「抄録」 認知症はせん妄発症の背景因子として重要なものであり, せん妄発症の最大の危険因子のひとつである. 認知症とせん妄は, 病状がオーバーラップしていることも多く, 鑑別診断は困難なことも多い. 認知症症例を対象としたせん妄のスクリーニングはなく, 今後の研究・開発が必要な分野である. また, せん妄の予防ならびに治療に対して, 薬物療法は高いエビデンスをもつものはない. せん妄発症時には, 抗精神病薬が使用されることが多いが, 必要時に最低限の期間・量での使用にとどめるべきである. なお, 抗精神病薬の予防投与は勧められていない. 認知症症例に特化した非薬物療法的せん妄予防法はないが, 高齢者に対して有効とされている多面的非薬物療法的介入法の体系的な導入が勧められる. 多面的な予防策が必要であり, リエゾンチームのケアの調整としての役割も期待される. |
