| Abstract | 「抄録」先進国では, 認知症は緩和ケア・エンドオブライフ・ケアの中心的課題となってきている. 重度〜末期認知症の代弁者による意思決定においては, 摂食嚥下障害の進行に伴う食支援や人工的水分・栄養摂取, さらには末期の肺炎の治療に関する意思決定が約半数を占めている. 本稿では, 重度〜末期認知症の身体的苦痛と緩和ケアの実際, それらの問題に伴う意思決定支援上の課題について解説する. |
