| Abstract | 「抄録」●終末期におけるせん妄は, 臨床では「身の置き所のなさ(restlessness)」や精神的な苦しみ(terminal anguish)と重なって生じる病態である. ●背景に痛み等の身体症状, 宿便や尿閉, アカシジア, ステロイドなどの身体的・薬物的な要因で生じていることも多く, 丁寧な鑑別が求められる. ●回復可能な原因を除去しても改善が図れないときには, 治療の目標をせん妄に伴う幻視等に伴う苦痛の緩和, 睡眠覚醒リズムの障害に伴う苦痛の緩和において対応を行う. ●家族の苦痛軽減にも注意をはらう必要がある. |
