| Abstract | 「はじめに」ネフローゼ症候群は, 持続する3.5g/日(あるいは3.5g/gCr)以上の蛋白尿および3.0g/dL以下の低アルブミン血症を呈する症候群である. 特発性(一次性)のものと, 他疾患に起因する二次性のものに分けられる. 二次性疾患は基本的には原疾患の治療が重要となる. 本稿では, 一次性ネフローゼ症候群, 中でも, ネフローゼ症候群の原疾患として, 比較的臨床現場で遭遇する微小変化型ネフローゼ症候群(MCD), 巣状分節性糸球体硬化症(FSGS), 特発性膜性腎症(IMN)の三疾患について, 診断ならびに治療に関して概説する. 「I. 各疾患における病態と診断」「1. MCD」小児の一次性ネフローゼ症候群の約90%, 成人の約40%を占める. 数日の経過で急激に病態が進行し, 突然の浮腫の出現・増悪, 体重増加を主訴に病院を受診することが多い. |
