| Abstract | 「はじめに」 肩腱板断裂は本邦で約1,900万人に存在すると言われ, 40歳代から出現し, 80歳代になるとその半数に存在する肩関節の代表的な変性疾患である. 外傷歴のない非外傷性腱板断裂の原因として, 腱板周囲からの外的要因と腱板自体の内的要因が考えられる. 外的要因には肩峰下インピンジメントや, 肩峰の形態, 骨棘, os acromiale, 大結節骨折の変形治癒などのほか, 軽微な外傷も含まれる. 一方, 内的要因には腱内の血流低下, 腱の加齢性変性, 腱の炎症などがあり, これらが複合的に重なって腱板断裂が発生すると考えられる. Neerは肩峰下骨棘など外因性インピンジメントが腱板断裂の原因となると主張していたが, 現在では加齢依存性の腱板機能不全に伴う骨頭上方化が肩峰との衝突を引き起こし, それによって腱板の一部の挟み込みが生じ, インピンジメント徴候の原因となる説が有力である. 「I. 肩腱板断裂の診断」 腱板断裂の痛みは, 肩だけに限局せず, 頚部から前腕まで放散する疼痛を認めることがあるため, 頚椎由来の疼痛と誤診されることがあるので注意が必要である. |
