| Abstract | 「はじめに」泌尿器科領域の感染症は, 臨床経過により「急性」と「慢性」, 基礎疾患のなし・ありにより「単純性」と「複雑性」, 感染部位により尿路感染症 (膀胱炎, 腎盂腎炎) , 性器感染症 (前立腺炎, 精巣上体炎) に分類される. これら臨床経過, 基礎疾患の有無および感染部位を組み合わせて診断名とする. 一般的に膀胱炎は発熱をきたさないため, 発熱をきたす泌尿器科領域の感染症としては, 主に尿路感染症では急性腎盂腎炎, 性器感染症では急性前立腺炎, 急性精巣上体炎が挙げられる. また, 最近では, キノロン耐性菌や, Extended spectrum β-lactamases (ESBL) 産生菌など, 尿路・性器感染症原因菌の薬剤耐性化が問題となっている. 発熱をきたす泌尿器科領域の感染症治療を行う上で, 原因菌の薬剤感受性に応じた抗菌薬選択も, 発熱診療の重要なポイントである. |
