| Abstract | 「はじめに」近年, 高齢化社会の進展に伴いサルコペニア・フレイルの概念が注目されている. 医療の進歩により世界で平均寿命自体は延伸しているものの, 様々な疾患を伴い, 要介護状態となるリスクの高い高齢者数は増加している. 骨格筋の量的・質的障害に特徴付けられるサルコペニアは要介護の高リスク状態であるフレイルへの進展の重要な要因である. また, 糖尿病ではサルコペニアが進行しやすくなり, また「インスリン作用の場」としての骨格筋量の減少は, 糖尿病を悪化させる要因となる. 一方で, 糖尿病患者は肥満を合併することも多く, 肥満に対する対応とともに, 痩せと強く関連するサルコペニアに対する対応とを合わせて行う必要もある. 高齢者のケアにおいて, サルコペニアのリスクを踏まえて治療を行うことは, 今後の糖尿病治療においてさらに重要になると考える. |
