| Abstract | 「はじめに」 肺炎は老若男女が罹患する疾患であるが, 特に高齢者においては罹患率, 死亡率ともに高く関わりの深い感染症である. 誤嚥性肺炎も含めると日本人の死因第3位の疾患であり, その死亡者の96%以上を65歳以上の高齢者が占めている. 近年, 肺炎の死亡者数が増加している原因は日本社会の高齢化によるものと推測される. 2020年の推計では65歳以上の高齢者の人口割合は29.3%と3割に迫っており, 今後も増加傾向が続くと推定される. 高齢者肺炎に対する対策が重要な課題となっており, 本稿では高齢者肺炎の外来診療について概説する. 「I. 高齢者肺炎の特徴」 高齢者は感染防御能や心身認知機能が低下しており, 感染症診療において健康成人と異なる特徴を有する. 一般に, 高齢者の肺炎では発熱や咳嗽などの特異的な症状が欠如していることがしばしばある. 特に発熱については18〜44歳では約85%で出現するのに対して, 75歳以上の高齢者では50%程度しか出現しないとする報告もある. |
