| Abstract | 「はじめに」 潰瘍性大腸炎に対して, 免疫抑制剤や抗TNF製剤療法などが新規治療法の有用性が報告され, 薬物療法が奏効し長期にわたり緩解維持できる症例も増えてきた. しかし, 内科的治療にて効果が得られない症例においては外科治療が必要となる. 実に20〜30%の潰瘍性大腸炎患者が手術を必要とすると報告されている. 外科手術の遅れは術後合併症を増やしてしまうため, 重症例や患者の予備能に応じて手術時期を決定しなければならない. また, ステロイドや免疫抑制剤を投与している場合においても慎重な対応が必要である. 本章では, 手術適応, 術式選択, 術後合併症に関して最新の報告をまとめて記載する. 「I. 手術適応」 「1. 手術の適応」 手術適応は, 絶対的適応と相対的適応に分けられる. 絶対的適応に関しては, 大腸穿孔, 制御不能な出血, 中毒性巨大結腸症, 大腸癌, またはHigh grade dysplasia, 内科的治療が奏効しない重症例があげられる. |
