| Abstract | 「はじめに」多発性筋炎(Polymyositis, PM), 皮膚筋炎(Dermatomyositis, DM)はおもに横紋筋に炎症が生じ, 筋力低下, 筋痛などをきたす自己免疫疾患であるが, 皮膚筋炎ではこれに加えて特徴的な皮膚症状を呈する. さらには高頻度に間質性肺病変を合併し, 時に重篤となる疾患である. 本邦におけるPM/DMの推定患者数は増加傾向であり, 平成30年度末の医療受給者証保持者数は22,195人である(難病情報センターHP). 男女比は1:3で, 成人では50歳代に好発する. 本稿では本症の臨床所見および診断から治療まで, 最近発表された治療ガイドラインの紹介を含め実際の診療に役立つように概説する. 「I. 症状, 身体所見」主要な症状は全身症状, 筋症状, 皮膚および関節症状である. 全身症状は発熱, 倦怠感, 体重減少などが出現する. 筋症状は本症の中心的症候で, 筋痛および筋脱力を呈し, 四肢の近位筋や体幹, 咽頭喉頭などに生じる. |
