| Abstract | 「はじめに」小児の免疫・膠原病外来では, 最も頻度が高い若年性特発性関節炎をはじめとして, 全身性エリテマトーデス(SLE), 皮膚筋炎, 強皮症, シェーグレン症候群など自己免疫疾患を対象として従来診療を行ってきた. しかしながら, 近年はこういった疾患に加え, 自己炎症疾患が非常に増えてきている. 広義なものを含めると自己免疫疾患より初診の患者数は多くなってきている. また, 外来での生物学的製剤の投与例も増えている. こういった慢性疾患は従来よりコントロールが良くなり, 成人へのtransition(移行期医療)も重要な課題となっている. 本稿では増加してきている小児期の自己炎症疾患を中心にエピソディックに記載する. 「I. 小児期発症SLEの現状」SLEの5分の一は小児期発症で, 軽微な尿所見異常でも腎生検で重症度を判断されたり, 免疫性血小板減少性紫斑病(ITP)でフォロー中に診断されたりする症例も増え, 治療介入が早くなってきている. |
