| Abstract | 「はじめに」現代の臨床医学において, 中枢神経の再生は未だ実用に至っていない. このため脳梗塞の治療については, リハビリテーションしか望めない時代が続いた. その後, 1990年代に非弁膜症性心房細動に伴う脳梗塞の再発予防に対してワルファリン, 非心原性脳梗塞の再発予防に対してアスピリンの有効性のエビデンスが示され, 二次予防を行うことができるようになった. また, 1995年に初めてrt-PA静注療法が海外で有効性が示され, 2005年に我が国でも使用可能となった. これにより発症早期の適切な治療によって不完全虚血部位であるペナンブラ (Penumbra) を救済することで脳梗塞の治療が可能な時代になった. 本稿では近年の研究結果をふまえ, 脳梗塞のプライマリーケアについてまとめた. |
