| Abstract | 「はじめに」日本における骨粗鬆症患者数は1,280万人と推定されている. 骨粗鬆症性骨折の発生件数は経年的に増加している. 骨粗鬆症治療薬の選択においては, 既存骨折はないが骨折リスクのある患者に対して骨折を生じさせない一次予防と既存骨折がある患者に対して再骨折を防ぐ二次予防に分けて考えることがポイントである. 骨粗鬆症の重症度に応じて薬剤を選択するとともに, 患者の嗜好やライフスタイルに適した剤形や投与間隔の治療によりアドヒアランスを向上させることが重要である. 骨粗鬆症の治療目的は, 骨粗鬆症患者のQOLを維持し, 健康寿命を延伸させることである. 「I. 骨粗鬆症の診断と薬物治療開始基準」原発性骨粗鬆症は, 脆弱性骨折の有無と骨密度から診断する. 脆弱性骨折がある場合は, 「大腿骨近位部骨折または椎体骨折がある」あるいは「その他の脆弱性骨折があり, 骨密度<80%YAM(young adult mean)」をもって骨粗鬆症と診断する. |
