| Abstract | 「はじめに」超高齢社会に突入したわが国では, 令和4年(2022年)の国民生活調査において, 腰下肢痛が高齢者の有訴率の第1位となっており, その主な原因の一つに腰部脊柱管狭窄症が挙げられる. 腰部脊柱管狭窄症とは, 脊柱管を構成する骨性要素や椎間板, 靭帯性要素などによって腰部の脊柱管や椎間孔が狭小となり, 馬尾あるいは神経根の絞扼性障害をきたして症状を発現した病態である. 日本における大規模前向き研究では, 対象者の9.3%が腰部脊柱管狭窄症に罹患しており, 特に高齢者では10%に及んでいたとの報告がある. 本症は, 急激に症状が進行することは少ないため重症例以外は保存療法が第一選択とされ, 改善が得られない場合は手術治療が考慮される. 治療を行う際は, 本症の病態や自然経過を十分理解して進めていく必要がある. 本稿では, 腰部脊柱管狭窄症の診断と痛みの評価, 治療(保存療法, 手術治療)について概説する. |
