| Abstract | 「I. 帯状疱疹後神経痛の病態」帯状疱疹後神経痛(postherpetic neuralgia:PHN)は, 帯状疱疹の皮疹が改善した後も痛みが残存し, 持続する病態である. 患者はその痛みを, 「ちりちり」, 「電気が走るような」, 「焼けるような」などの様々な表現で医療者に伝える. PHNは, ウイルスにより損傷した神経に異所性発火, つまり痛覚受容器を介さずに本来は興奮が生じない箇所から活動電位(インパルス)が発生することで起こる代表的な神経障害性疼痛である. 疼痛の伝達機構としては, 末梢神経の損傷部位での異所性発火で生じたインパルスが脊髄後角から脊髄視床路を経て中枢に伝達され, 大脳で痛覚を感じるというプロセスが想定される. 神経障害性疼痛はインパルスによる電気的な痛みであるため, 抗炎症鎮痛薬である非ステロイド性抗炎症薬(Nonsteroidal antiinflammatory drugs: NSAIDs)の効果は無い. |
