| Abstract | 「はじめに」術後痛管理は痛みによる苦痛を軽減するだけでなく, 痛みによる体内ストレス反応を低減することで, 呼吸器・循環器を含めた術後合併症の予防につなげ, 早期離床を目指すものである. さらに, 術後痛という急性痛のうち, 術後長期に渡り痛みが続く術後遷延痛は20〜56%の患者が経験し, 2〜10%は6ヵ月後も, 約2%は12ヵ月後も激しい神経障害性疼痛に苦しんでいる. 特に, 下肢切断術, 開胸術, 胆嚢摘出術, 乳房切除術などは高リスクである. この激しい術後遷延痛の多くが術後痛からの移行と考えられ, 急性痛を制御することは慢性痛の予防にもつながるため, 術後疼痛管理は重要である. 「I. 術後痛管理の考え方」「1. Multimodal analgesia」術後痛は主に侵害受容性の疼痛であり, これまで硬膜外鎮痛を主体とした局所麻酔法にNSAIDs, オピオイドを併用した鎮痛が主たるものであった. |
