| Abstract | [Points] ●基質特異性拡張型βラクタマーゼ(ESBL)産生腸内細菌科細菌の治療薬として, 全身状態が安定していれば, カルバペネム系抗菌薬以外にセフメタゾールなども治療の候補になりうる. ●経静脈的抗菌薬から経口抗菌薬に変更を提案するときに, 南オーストラリア州政府のガイドライン(STOP)を参考にすることができる. ●タゾバクタム/セフトロザンは多剤耐性緑膿菌などの耐性菌治療の重要な候補薬剤であるが, 乱用するべき薬剤ではなく, 感受性試験を行っていない施設では使用は控えたほうがよい. ●Clostridioides difficile(CD)腸炎の治療では, わが国では, 軽症例ではメトロニダゾールが候補となる. ●市中肺炎治療時のセフトリアキソン投与量は, 重症肺炎でなければ1g/日でもよい. ●バンコマイシンの投与調整は, 現在トラフ値を指標としているが, 将来的には, 重症methicillin-resistant Staphylococcus aureus(MRSA)感染症に対する投与調整はAUC/MICBMD法が推奨されるようになるかもしれない. ●グラム陰性菌の菌血症は, 10〜14日間が標準的治療期間だったが, 合併症を伴わない経過良好な症例では, 7〜10日間の治療も選択されうる. ●感染性心内膜炎の治療は, 経静脈的投与が原則ではあるが, 病因微生物や患者背景によっては経静脈的な治療後に経口抗菌薬で治療を完遂するというオプションも考慮されるようになるかもしれない. ●妊婦に推奨されるワクチンとしては, インフルエンザワクチンとともに, 海外では妊娠27〜36週に接種する成人用三種混合ワクチン(Tdap)がある. |
