| Abstract | [Points] ●アミオダロンはK+チャネル遮断作用に, Na+チャネルやCa2+チャネル遮断作用, 交感神経抑制作用, 甲状腺機能修飾作用などもあわせもつマルチチャネル遮断薬である. ●アミオダロンは肝代謝型で活性代謝物(デスエチル体)を有し, 脂肪組織への分布が大きく, 消失半減期が14〜107日と長い. ●突然死予防, 心房細動の除細動・再発予防に用いられ, 心不全・低心機能例にも使用できる. ●アミオダロンの心外副作用として, 肺毒性〔間質性肺炎と甲状腺障害(特に甲状腺機能亢進症)〕には注意が必要である. ●長期治療では抗不整脈効果を維持しながら, 低用量化を進めていくのが望ましい. |
