Title C型肝炎の治療戦略 - C型肝炎治療ガイドラインの活用 (第7版と第8版の改訂ポイント)

Abstract [Points] ●非代償性肝硬変に対してはソホスブビル / ベルパタスビル配合錠12週投与が選択肢となるが, Child-Pughスコア13〜15点の症例に対する安全性は十分担保されていないため, 肝臓専門医によって治療方針が決定されるべきであり, 投与の場合には極めて慎重な経過観察が望ましい. ●直接作用型抗ウイルス薬 (DAA) 前治療不成功例に対する再治療は, グレカプレビル / ピブレンタスビル配合錠12週投与, またはソホスブビル / ベルパタスビル配合錠+リバビリン24週投与が推奨される. DAA前治療不成功であった非代償性肝硬変に対する効果が確立したレジメンはない. ●C型肝炎ウイルス (HCV) が排除されると新規の肝発がんリスクは低下するが完全には消失しないため, 高齢, 男性, 線維化進展, アルコール摂取, 肝脂肪化, 糖尿病, および治療後のAFPやM2BPGiなどを参考にして, 定期的な肝がんに対するスクリーニングを行う. ●肝移植後のC型肝炎再発には抗ウイルス治療を行うことが推奨され, 特に胆汁うっ滞性線維性肝炎 (FCH) に対しては, 早期に抗ウイルス治療を導入する必要がある. ●HCV陽性妊婦の診療は「産婦人科診療ガイドライン - 産科編2017」, 小児C型肝炎の診療は「C型肝炎母子感染小児の診療ガイドライン」に準じて行う. ●12歳以上の小児ではDAA治療の適応があり, 12歳未満でも慢性肝炎または線維化が強い症例では治療を考慮する. グレカプレビル / ピブレンタスビル配合錠は12歳以上のC型慢性肝炎に対して保険収載されている.
