| Abstract | 「Answer」第1段階ではロラゼパム0.1mg/kg, もしくは固定用量4mgを最大2mg/分で静脈内投与し, 効果を評価した後, 追加投与が必要かどうかを決定します. 発作が続いている場合はこれを繰り返します. ロラゼパムが利用できない場合は, ジアゼパム0.15mg/kg, もしくは5〜10mgを5mg/分で静脈内投与します. 第2段階では非ベンゾジアゼピン系抗けいれん薬 (ホスフェニトイン, またはレベチラセタム) による治療が推奨されます. 難治性のてんかん重積の場合はミダゾラム, プロポフォール, チオペンタール, チアミラールによる全身麻酔療法を導入します (第2段階以降の用量・用法については図1を参照) . |
