Abstract [Points] ●敗血症における初期治療では, 感染臓器や患者背景などから起因菌を想定し, それらを過不足なくカバーする抗菌薬を選択する. ●特に初期治療では耐性菌リスクを適切に評価し, それに応じて広域抗菌薬を使い分ける. ●「日本版敗血症診療ガイドライン2020」では, 感染臓器ごとに頻度の多い起因菌と初期治療に使用する抗菌薬についてまとめている. ●敗血症時は腎機能が短時間で劇的に変化するため, 血清クレアチニン値だけでなく, その推移や尿量などを含め総合的に評価する.
