| Abstract | [Points] ■片頭痛は, 日常生活に支障のある頭痛発作を繰り返し, 悪心・嘔吐, 光過敏, 音過敏などの随伴症状を伴う神経疾患である. 個々の患者の生活の質は大きく障害され, 社会全体に与える経済的影響も非常に大きい. ■頭痛診療の目的は, 正確な頭痛診断を行うことと, 可能な限りの手段を尽くし頭痛患者の頭痛に伴う疾病負担 (burden) を軽減することである. ■頭痛診療では, くも膜下出血などの緊急性の高い二次性頭痛を鑑別することが第一歩である. 二次性頭痛の鑑別は問診, 身体所見, 神経学的所見を丁寧にチェックし, 画像検査や血液検査, 髄液検査などを組み合わせて進める. ■慢性的な頭痛のために医療機関を受診する患者の多くは片頭痛である. 一次性頭痛の鑑別では, 片頭痛があるかどうかを意識して問診する. ■頭痛診療では, 戦略的かつ適切な急性期治療薬と予防療法の選択, きめ細かい生活指導を実践する必要がある. |
