| Abstract | [Points] ●アンジオテンシン受容体ネプリライシン阻害薬(ARNI)のサクビトリルバルサルタンは, 1分子中にアンジオテンシンII 受容体拮抗薬(ARB)のバルサルタンとネプリライシン阻害薬のプロドラッグであるサクビトリルを1 : 1で結合含有させた化合物である. ●ネプリライシン阻害により増加する内因性ナトリウム利尿ペプチドは多彩な心血管保護作用を有する. ●左室駆出率(LVEF)の低下した心不全(HFrEF)を対象としたPARADIGM-HF試験において, ARNIのACE阻害薬エナラプリルを上回る予後改善効果が証明された. ●症状を有するHFrEF患者に対して, ACE阻害薬からARNIへの切り替えはガイドラインにおいて推奨されている. ●LVEF の保たれた心不全(HFpEF)を対象としたPARAGON-HF試験において, ARNIはARBのバルサルタンに対して予後改善傾向を認めたが, 統計学的には有意でなかった. |
