| Abstract | [Points] ●メトトレキサート(MTX)は関節リウマチ(RA)治療のアンカードラッグである. ●RAと診断されて予後不良の症例には, 禁忌がなければMTXを第一選択薬として投与する. ●MTXは通常週6〜8mgから開始して, 効果をみながら週10〜12mgまで, 4週ごとに週2mgずつ漸増するが, 予後不良の非高齢者ではより速やかに増量できる. ●MTX単剤で効果不十分な場合は, MTXをアンカーとして従来型合成抗リウマチ薬(csDMARD), 生物学的製剤(bDMARD)との併用療法を行う. ●MTXの副作用として, 免疫抑制下で発生する感染症とリンパ増殖性疾患が増加傾向にある. 特に, 長期内服の高齢者では注意が必要ある. |
