| Abstract | 「症例」78歳, 男性, 身長172cm, 体重52kg 【既往歴】精神科受診歴なし, 逆流性食道炎, 2型糖尿病 【家族歴】精神科受診を含め, 特記すべきことなし 【生活歴】喫煙:なし, 飲酒:機会飲酒 【現病歴】肺がんの手術のために2日前に入院し, 昨日, 手術を終えた. 昨夜はICUに入室し, デクスメデトミジンを使用し問題なく眠れていたようである. 本日, 呼吸器外科病棟に帰室, 持参の内服薬を再開し, 日中は問題なく過ごせていたとのことである. 夕食後から点滴ルートをいじるような行動があり, 意識も少しぼんやりしている様子であった. 消灯時間になっても就寝できず, 約束指示に基づき不眠時頓服を投与したが, その1時間後に点滴ルートを引き抜いて「帰る」と大騒ぎしたため, 落ち着かせるのに良い薬はないかと薬剤部当直に電話があった. 【使用薬】「1)持参薬」ファモチジン錠20mg 1回1錠 1日2回 朝夕食後 エチゾラム錠1mg 1回1錠 1日3回 毎食後(手術への不安のため1カ月前から服用開始) |
