| Abstract | 「症例」80歳代, 男性, 身長173cm, 体重52kg 【既往歴】認知症(内服加療なし) 自転車運転中に自動車と接触し, 意識消失のため救急に連絡. 救急隊接触時, 頭部・前胸部挫創から出血あり. 呼びかけ・刺激に反応なく, 重症頭部外傷が疑われ当院救命センターへ搬送となる. 救急外来での状況, 処方などは以下のとおり. 【来院時現症】気道開通, SpO2 100%(O2 10L/分), 呼吸数 25回/分, 血圧 182/98mmHg, 脈拍 102回/分, 心電図モニタ 正常洞調律, JCS III-300, GCS E:1, V:1, M:1 合計3点, 瞳孔不動, 瞳孔径:右5mm, 左3.5mm 救急外来にて気管内挿管され, CT施行予定であったがけいれんあり. 配置薬のジアゼパム注射液10mgを投与したがけいれん頓挫せず, 以下が夜勤帯に処方となり, 研修医が調剤室に取りにきた. |
