| Abstract | 「Answer」カリウム除去は血液透析が迅速かつ確実ですが, 準備と説明に時間を要します. 溶血など偽性高カリウム血症が除外され, 筋脱力, 心電図異常(テント状T波など)のような症状がある場合には緊急性が高いと判断する必要があります. 配合変化に注意しながら(炭酸水素カルシウムで沈殿)緊急時の治療薬としてグルコン酸カルシウムを用いますが, 代謝性アシドーシスを合併している状況では炭酸水素ナトリウムの投与も考慮します. 効果発現は遅れますが, グルコース・インスリン療法(ブドウ糖液にレギュラー・インスリンを混注)も行われます. 緊急でなければフロセミド, ポリスチレンスルホン酸ナトリウムやポリスチレン酸カルシウム, ジルコニウムシクロケイ酸ナトリウム, β刺激薬の吸入剤などを用います. |
