| Abstract | [Points]●腎臓は心機能低下の影響を最も受けやすく, CKDは心不全に合併する臓器障害としては臨床的に最もよく遭遇する. ●腎機能低下は, 体液量の増加, 尿毒症性物質の蓄積, 炎症性サイトカインの増加などを介して, 前負荷の増大や心筋リモデリングの進行に関連している. ●ACE阻害薬, ARB, SGLT2阻害薬には腎保護作用があることが知られているが, ACE阻害薬, ARBはCKDステージ4以上の症例では, 急激な腎機能悪化や高K血症を来すことがあり, 十分な注意が必要である. ●ミネラルコルチコイド受容体拮抗薬(MRA)もCKDステージ4以上では高K血症のリスクが高くなるため, 慎重なモニタリングと減量や休薬を検討することが望ましい. |
