| Abstract | 「Key Point」 ●過敏性腸症候群(IBS)は明らかな器質的疾患がないにもかかわらず, 腹痛や便通異常を来す疾患である. ●患者数が多く, QOLや労働生産性を著しく低下させる重要な疾患であるため, 適切な治療が必要である. ●IBSの病態はストレスに対する過剰応答が要因となって, 消化管運動機能異常や内臓知覚過敏などが生じ, 症状が発現する. ●過剰応答を修飾する因子として幼少期や思春期の環境, 過去の消化管感染症の既往, 遺伝子要因, 最近では腸管粘膜透過性の亢進や腸内細菌叢の異常, 腸管粘膜の微小炎症などが重要な役割を果たしていると考えられている. ●下痢型IBSの薬物治療の第一段階として, 5-HT3拮抗薬であるラモセトロンが2014年に発売され, 「機能性消化管疾患診療ガイドライン2020-過敏性腸症候群」でも推奨されている. |
