| Abstract | 「到達目標」●心不全患者におけるSGLT2阻害薬のエビデンスについて理解する. ●心不全患者の薬剤性増悪因子について理解する. ●現時点でのSGLT2阻害薬の心不全患者に対する使い分けを理解する. 第13回は, SGLT2阻害薬の心不全患者への使い方について理解し, 明日からの臨床業務に活かそう! 「症例」72歳, 男性, 身長 167.2cm, 体重 89.1kg(BMI 31.9) 「現病歴」入院1週間前より労作時の呼吸苦があり, ここ1カ月で体重が10kg増. 自宅のSpO2モニターで普段は98%程度であるが, 70%台になったため救急受診. 水分や塩分のとり過ぎはなく, 上気道症状もなし. 怠薬もなかった. 「既往歴」56歳:2型糖尿病, 不安定狭心症 70歳:前立腺肥大 「家族歴」詳細不明 「身体所見」血圧 146/76mmHg, 脈拍 95回/分(整), 呼吸数 18回/分, SpO2 94%(room air), 頭頸部:結膜貧血なし, 黄染なし, 胸部:心音 整 雑音なし, 両肺呼気終末喘鳴あり, 胸水あり, 胸痛なし, 腹部:腹痛なし, 嘔気・嘔吐なし, 下血なし, 四肢:両下腿浮腫あり |
