| Abstract | [Key Point] ●抗インターロイキン (IL) -12/23抗体であるウステキヌマブは, 有効性, 安全性, 利便性に優れた薬剤であるが, 効果発現は比較的緩徐である. ●ウステキヌマブは, 潰瘍性大腸炎 (UC) では, 急いで病勢をコントロールしなければならないような症例を除いて, ほぼ全例に適応になるのに対し, クローン病 (CD) では抗TNFα抗体の有効性が非常に高いため, バイオナイーブでの使用に関しては病態を考慮する必要がある. ●ウステキヌマブの抗原性は低く, 抗薬物抗体の発現率が低い. そのため二次無効になりにくく, 免疫調節薬との併用も必須ではない. ●抗IL-23抗体は, 複数の製剤が現在治験段階にあり, 近い将来にUC, CDともに使われるようになる見込みである. その製剤特性やウステキヌマブとの効果の比較などは今後明らかにされていくものと思われる. |
