| Abstract | 「Key Point」●主な不安症として, パニック症, 社交不安症, 全般性不安症がある. DSM-5では不安症から, 強迫症と心的外傷後ストレス障害が独立している. ●いずれの不安症に対しても抗うつ薬による薬物療法が有効であるが, 海外のガイドラインでは, まず認知行動療法が推奨されるものも多い. ●抗うつ薬としては, 主に選択的セロトニン再取り込み阻害薬(SSRI), セロトニン・ノルアドレナリン再取り込み阻害薬(SNRI)が使用されるが, 一部推奨されていない薬剤もある. 日本では保険適用外となる薬剤も推奨されている. ●抗うつ薬投与の原則は, 効果と副作用についての十分な説明と同意のもと, 十分量を十分期間投与して効果を判定することである. 効果のない薬剤を漫然と使用することは望ましくない. |
