| Abstract | 「Key Points」●意識障害の原因鑑別には意識レベル, 持続時間, 随伴する症状の評価が重要である. ●重篤な意識障害が持続する場合, くも膜下出血や髄膜脳炎, 広範な脳出血, 脳梗塞などの鑑別のため, 頭部画像検査(MRI, CTなど)は必須である. ●失神は心原性失神, 神経調節性(反射性)失神, 起立性低血圧症に大別されるが, いずれも短時間で生じる一過性の脳虚血, 低灌流により生じ, 失立を伴いやすく骨折, 打撲などの外傷をきたすことが多い. 持続は数秒から数十秒であり, 大量の発汗などを伴うことがあるが, 神経学的局所症状をきたさない. ●けいれんは失神後に生じることもあるが, 多くはてんかん発作に伴うため, 脳波などの検査が必要である. ●薬物中毒や精神疾患, 過換気症候群やヒステリー発作によるけいれん, 意識障害の鑑別はしばしば困難なことがある. |
