| Abstract | 「Answer」脳梗塞の臨床病型としては, アテローム血栓性脳梗塞, 心原性脳塞栓症, ラクナ梗塞, その他の4つに分けられ, それぞれ再発(二次)予防のために推奨される薬剤が異なる. 心原性脳塞栓症では, 直接作用型経口抗凝固薬(DOAC), ワルファリンなどの抗凝固療法を用いる. 一方, アテローム血栓性脳梗塞やラクナ梗塞を中心とした非心原性脳梗塞においては, 抗血小板薬を用いる. わが国で用いられる抗血小板薬としては, アスピリン, シロスタゾール, クロピドグレル, プラスグレルがある. 脳梗塞急性期, またハイリスク症例などにおいては, 2剤併用療法が行われることもある. 「解説」脳梗塞は, 何らかの原因で脳血管が閉塞し, 脳組織のエネルギー破綻をきたすことによって生じる. 臨床病型としては, アテローム血栓性脳梗塞, 心原性脳塞栓症, ラクナ梗塞, その他の4つに分類される. |
