| Abstract | 「はじめに」眼窩炎症を引き起こす疾患群を眼窩炎症症候群として捉えた場合, 眼痛, 眼瞼腫脹, 複視などの眼症状を有するといった共通点がある一方で治療が全く異なる場合もある. したがって, どのような疾患が眼窩炎症症候群の原因疾患となるのか, また各疾患がどのような特徴を有するのかを把握しておく必要がある. 本稿では眼窩炎症性病変のなかでも代表的な甲状腺眼症の特徴について言及し, 鑑別として挙げられるその他の眼窩炎症症候群について解説していく. 「甲状腺眼症とはどのような病態か」バセドウ病の3徴として眼球突出は古くから知られているが, 甲状腺眼症の病態が明らかにされてきたのはつい最近のことである. 甲状腺眼症とは, 甲状腺刺激ホルモン(TSH)受容体抗体が眼窩内脂肪や外眼筋に存在し, それが標的となって自己免疫性炎症が起こる病態である. |
