| Abstract | 「はじめに」視神経炎(optic neuritis;ON)と前部虚血性視神経症(anterior ischemic optic neuropathy;AION)は急性, 片眼性視神経症の代表的疾患である. 診断は症状, 臨床所見, 経時的変化などから総合的に行われるが, 臨床所見は類似する点も多く診断に難渋することも少なくない. 視神経萎縮期では特異的所見が乏しくなり, 両者の鑑別がさらに困難になる. ONにおけるステロイド治療は視機能改善を早める一方で, 非動脈炎性AION(non-arteritic AION;NAION)では有効ではなく治療の反応性は大きく異なる. 臨床的特徴とともに症状経過に基づいて両疾患を適切に診断することは治療方針の決定や再発リスク, 将来的な合併症リスクを推定するうえで重要となる. また, ONと診断したのちには早い段階で典型的ONと非典型的ONとの鑑別が必要となる. |
