| Abstract | 「はじめに」IMPELLA(R)とは, カテーテルの先端に小型軸流ポンプと脱血・送血カニューレを装備した定常流のポンプカテーテルのことを言い, 心原性ショックなど薬物療法抵抗性の急性心不全の患者に使用される. デバイスの特性は, 経皮的または経血管的に左心室内に挿入したIMPELLA(R)補助循環用ポンプカテーテルが吸入部から血液を脱血し, 上行大動脈内にある吐出部に順行性に送血する. これにより左室拡張期容量・左室拡張末期圧が低下し, 心負荷軽減と心筋循環改善が期待されている. 先行研究では, 心原性ショック時のIMPELLA(R)使用の安全性と実用性が示され, 本邦では2017年から補助循環デバイスとして普及している. 一方, IMPELLA(R)作動中の確立されたガイドラインはなく, 看護に関する文献も少ないのが現状である. |
