| Abstract | 「帯状疱疹とその発生機序」帯状疱疹はヘルペスウイルス科に属する水痘・帯状疱疹ウイルス(varicella-zoster virus, VZV)が再活性することで起こる疾患である. 片側の支配神経領域に一致する痛みと小水疱の帯状に出現する集簇が特徴である. 頭頸部領域に発症すると眼科合併症やハント症候群(Ramsay Hunt syndrome)や, まれに脳炎や髄膜炎を起こすこともあるため, 帯状疱疹は皮膚だけに起こる疾患ではない. 皮疹が治まった後, 長期間, 痛みが続く帯状疱疹後神経痛(postherpetic neuralgia, PHN)は, 患者のQOLを大きく低下させる. 「帯状疱疹のリスク因子」帯状疱疹は既往歴に水痘がある場合, 誰でも発症する可能性があるが, 高齢であるほどその発症する危険性が高くなる. そのほか, 免疫状態や基礎疾患は帯状疱疹の発症と関連する. 日本で, 2001〜2007年における, 17疾患, 55,492人の患者の帯状疱疹発症の有無をカルテベースで検索した単施設での解析報告がある. |
