| Abstract | 「CASE」58歳, 男性. 既往に2型糖尿病, 高血圧があり, 近医で内服治療中である. 1週間前から微熱と呼吸困難があった. 昨日から症状が増悪し, 意識障害も認められたため, 当院救命救急センターに救急搬送された. 来院時, 「あーうー」という発声を認めたが, バイタルサインは, 意識レベル:GCS E2V2M4(合計点8), 呼吸数32回/min, 脈拍124回/min, 血圧67/36mmHg(平均動脈血圧47mmHg), 体温38.2℃, SpO2 84%(リザーバーフェイスマスク10L/min)であった. 毛細血管再充満時間は4秒以上であり, 末梢冷感および冷汗, 網状皮斑を認めた. 心雑音は聴取しなかったが, 右下葉を中心に両肺野にCoarse crackleを聴取し, チアノーゼを認めた. 「必ず確認すべきことはこれ!」(1)低酸素血症の症例, 敗血症を疑う症例ではER到着後迅速に血液ガスをとる. (2)ショックを伴う症例, 低酸素を伴う症例では超音波検査で診断を行う. |
