| Abstract | 敗血症患者では"Sepsis is an emergency"を理解し, 常にスピード感をもってケア・治療を行えるよう, 患者のアセスメント・マネジメントが重要です. 本稿では事例を通して循環管理, 呼吸管理, 体温管理, せん妄ケア, 集中治療後症候群(post intensive care syndrome;PICS)に焦点を当てています. そのほか, 早期リハビリテーション, 早期栄養管理, 血糖管理, ICUダイアリー, 睡眠ケアなどの視点もあります. ガイドラインの一読をお勧めしますが, あくまでも道標であり, 各施設状況に応じた看護が求められます. 患者の最良な社会復帰に向けgood practiceを目指しましょう. 「症例提示」A氏, 60歳代男性. 妻と二人暮らし. 2日前より右腰背部痛と発熱があり, 市販薬を服用し自宅で安静にしていた. 普段の起床時間となっても起きてこないため妻が様子を見にいくと2階廊下に倒れており, 救急要請し搬送. 尿路感染症, 急性腎盂腎炎による敗血症の診断でICUへ入室となった. |
