| Abstract | 「●ERで抗菌薬・抗ウイルス薬を使用するのは?」 ・細菌感染症(咽頭炎/扁桃炎, 尿路感染症, 細菌性肺炎)の治療 ・ウイルス感染症(インフルエンザ感染症など)の治療 「●覚えておきたい薬剤」 「●薬剤別 ERナース必修ポイント&観察ポイント」 「アミノペニシリン」 [適応] 細菌性咽頭炎/扁桃炎 [必修ポイント] ◇細菌性咽頭炎/扁桃炎の原因として多い溶血連鎖球菌に効果がある. ◇製剤は静脈注射(アンピシリンナトリウム)と内服薬(アモキシシリン)が存在する. アモキシシリンは内服の吸収が良く, 経口投与量の74〜92%が全身に移行する. 帰宅が可能な細菌性咽頭炎ではアモキシシリンが処方されることが多い. 飲水が困難な場合や入院が必要な場合はアンピシリンナトリウムの静注が選択される. ◇半減期が短く, 1日に複数回の投与が必要である. 腎機能正常例では, アンピシリンナトリウムは2gを1日4回(髄膜炎では6回), アモキシシリンは1回250〜500mgを1日3回で使用する. |
