| Abstract | 「I. 疾患の概要」頚椎変性疾患は, 加齢に伴い椎間板の退行変性が原因で, 椎間板腔狭小化, 骨棘の形成, 椎間関節の変化, 後縦靭帯, 黄色靭帯の肥厚などに起因する疾患の総称で, 代表的なものは, 頚椎症, 頚椎椎間板ヘルニア, 頚椎後縦靭帯骨化症(ossification of posterior longitudinal ligament:OPLL)などが挙げられる. 「A. 頚椎症」50歳以上の中高年に多く発症し, 頚部痛や肩こりなどの局所症状を呈し, この症状に脊髄圧迫による巧緻運動障害や歩行障害などの脊髄症状を呈する頚椎症性脊髄症と神経根圧迫による上肢, 肩甲骨部への放散する痛み, しびれなどの神経根症状を呈する頚椎症性神経根症に分類されるが, 両者が合併することもある. 「B. 頚椎椎間板ヘルニア」頚椎の椎間板の線維輪が破れて, 脊柱管内, 椎間孔内に椎間板組織の一部が脱出した病態である. |
