| Abstract | 「はじめに」臨床の現場にいるときに, 患者が息が苦しいといえば呼吸困難, 呼吸困難といえば呼吸器疾患という感覚でケアにあたっていませんか? それではもったいないです! 呼吸困難はさまざまな危機的状況のサインの一つであることも多いです. 呼吸困難のメカニズムを理解し, 正しくアセスメントすることで的確なケアにつなげることができます. まずは本稿で呼吸困難の全体像を把握して, ファーストタッチしたときの診療フローを理解してもらい, 各論で疾患別の診療に関して解説していきます. 「正常な呼吸とは?」呼吸困難を認識するためには, 正常な呼吸を知っておく必要があります. 「観察ポイント」まずは安静時の呼吸を観察するわけですが, 所見をとる際に大切なのは, 患者に気づかれないように少し離れたところから行うことです. その次に診察, 会話をする中でさらに呼吸パターンの変化を観察していきます. |
