| Abstract | 「はじめに」1957年Kaplanが膝蓋骨と大腿骨をつなぐ靱帯の存在を初めて報告した. その後, WarrenとMarshallが1979年に発表した膝内側の3層構造の論文の中に内側膝蓋大腿靱帯(medial patellofemoral ligament:MPFL)という名称が明記されるが, その重要性は明らかではなかった. 1993年にConlanらが膝蓋骨の外方偏位に対するprimary restraintとしてMPFLが約50%の制動を担っていると報告してから, 膝蓋骨脱臼に対するMPFLの重要性が注目されるようになった. また, NomuraらがMPFLは膝蓋骨脱臼においてほぼ全例で断裂していたと報告してから, MPFL再建術は膝蓋骨脱臼に対する観血的治療の第一選択となりつつある. 本稿では, MPFL再建術におけるコツとピットフォールについて解説する. |
