| Abstract | 「本手術の基礎知識」腱板断裂のうち, 肩甲下筋腱と棘上筋腱の断裂は前上方断裂と呼ばれ, 後上方断裂と比較しtransverse force coupleの破綻を呈するため保存加療では代償作用がきかず求心位がとれないことが多く, また上腕二頭筋長頭腱の肥大, 脱臼, 亜脱臼などとも関連しているため, 比較的手術加療を行うことが多い. 手術は関節鏡を用いて行い, 棘上筋腱から肩甲下筋腱に連続する構造物, いわゆるcommaを同定することが重要である. また従来肩甲下筋腱はsingle row法で修復することが多かったが, 再断裂率が9〜35%と報告されている. 近年suture bridge法は後上方断裂で広く行われるようになり, 大結節のfootprintを幅広く面で押さえつけることが可能なため, 再断裂率がsingle row法より低いことが報告されている. 肩甲下筋腱もsuture bridge法で修復することで再断裂率が格段に下がったと報告されている. |
