| Abstract | 「はじめに」後果骨折や天蓋骨折では後果の骨片は比較的大きな骨片であることが多く, また骨幹部の骨折を見ると比較的単純な骨折であることが多い. また, 後果骨片(Volkmann骨片)は後上方へ転位をきたしていることが多く, 一般的には整復後スクリュー固定が選択されるが, 整復不良での固定となることもある. ピロン骨折では後果にかかる骨折は単純骨折であり, 比較的大きな骨片が残っていることが多いことから, 後外側での後果の整復固定を行い, それに合わせるように前方からの固定を追加することでより正確な整復が可能となるという報告もある. また前面の軟部組織の損傷が強い症例などではさまざまな他部位のプレートを流用することで脛骨後方からの内固定が可能であるという報告もある. |
