| Abstract | 「本術式の基礎知識」人工膝関節置換術(TKA)において大切なことは, 適正な下肢アライメントと安定性確保のためのsoft tissue balancingおよびインプラントの正確な設置である. Soft tissue balancingの術式には(1)gap balancing techniqueと(2)measured resection techniqueの2つがある. gap balancing法はまず大腿骨後顆を骨切りし長方形の屈曲gapを作製し, その後同じ大きさのgapを伸展位で作るように大腿骨遠位部を骨切りする方法である. しかし後十字靱帯(PCL)を切離するposterior stabilizer design(PS型)では屈曲gapが拡大するために, 同じgapを伸展位で求めるとjoint lineが上昇すると言った欠点があった. それを改良したのが先に大腿骨遠位を骨切りし伸展位でgapを作製し, その後同じ大きさの屈曲gapを作るmodified gap balancing法であり, 現在では主流となっている. |
