| Abstract | 「はじめに」 アキレス腱断裂は近年増加傾向にあり, スポーツ活動に関連した受傷は60〜80%程度とされている. 本邦ではバレーボールやバドミントン, テニスといった競技において受傷することが多い. 一方, 末期腎不全患者において受傷リスクが上昇するという報告もあり, またニューキノロン系の抗菌薬や副腎皮質ホルモンなどがアキレス腱断裂の危険因子であるという報告もある. スポーツ選手における受傷も多く, 治療方法としては早期復帰を期待して観血的治療が行われることも多い. しかし, 近年では厳密な治療プロトコルに沿った保存療法で良好な治療成績を収めたとする報告も散見される. 本稿ではアキレス腱断裂の保存療法に関して適応や一般的な治療方法, さらに社会活動やスポーツ活動への復帰を目指したリハビリテーションについて詳述する. 「アキレス腱断裂に対する保存療法の特徴」 1968年にLea & Smithがアキレス腱断裂の保存療法に関して報告して以来, 本邦においても数多く報告されている. |
